Step 2 : Medical Staff Referral
Ask your Social worker, Doctors Office, or Nurse to go to www.meredithsmiracles.org
Fill out Medical Staff Referrals Form each time you have an
Emergency, doctor Appointment, NICU Stay, Surgery, Cancer Treatments, etc.

Provide a picture of your photo ID with address that you live in one of our counties.
Age requirements
Birth to 18 years old
Must be referred out of your area.
We help with non-medical expenses when you have to travel outside your area. Such as gas, lodging, and meals.

We have to have our Meredith's Miracles APPLY FOR ASSISTANCE Form filled out for each trip.
